Nondiscrimination Policy

Greenberg Foot and Ankle, LLC follows all state and federal policies on discrimination.  On May 18, 2016, the U.S. Department of Health and Human Services (HHS) published a final rule implementing Section 1557 of the Affordable Care Act.  Section 1557 and the new rule, entitled “Nondiscrimination in Health Programs and Activities,” prohibits discrimination on the basis of race, color, national origin, sex, age, or disability in certain health programs and activities receiving federal financial assistance (excluding Medicare Part B).  “Covered Entities” under this rule has the same meaning as defined by HIPAA:  healthcare providers, payers, and clearinghouses.  The policy on interpreters in Ohio can be obtained by clicking on the link button below.

New Patient Info

Welcome to our practice!  If you have not yet scheduled an appointment, call (937) 938-6444 and our staff will be happy to assist you.

Once you have an appointment scheduled, you will need to read our new patient forms and complete the information requested.  You can download the forms by clicking the button below.

Items to bring to your first appointment:

  • Valid form of ID.

  • Current insurance card.

  • Method of payment (if you have a copay).

  • Completed registration forms.

  • Parent/Guardian for patients under 18.

Please note that due to building and office policy you are not to bring any other people with you to your appointment unless it is medically or legally necessary.  In cases when an assistant, parent or guardian is necessary, only one person can accompany the patient.  This policy is in place to help minimize the exposure of all people coming and going in the building and in the office.
